IRB Services Updates QI / PI Application Form to version 3

IRB Services posts new “smart form” version of our QI/ PI Application Form: Qualified Investigator Submission Form final v3 100601.dot. Form is editable and can be completed electronically using Microsoft Word. Please not that client’s submitting through our online system (RED) can fill out the form directly online and do not need to download the Word version.
